Consent to Health Care

Home Visit Medicine: Acupuncture and Chinese medicine provide greater accessibility for the broader community, helping individuals achieve freedom while maintaining their independence.

Our team of highly qualified, experienced and accredited allied healthcare professionals will do our best to provide you with the highest quality natural medicine intervention and care. Our team are members of associated professional organizations and practice according to their outlined code of conduct and ethics.

Our practitioners are consistently updated with continuing education and changes to the healthcare system here in Australia. This is why our healthcare team will be doing our best to make your experience enjoyable, informed, beneficial and safe.

1. I have had an opportunity to outline my healthcare needs to my healthcare provider. Whereby a detailed diagnosis, and a proposed healthcare management plan will be outlined and implemented upon undertaking any healthcare services offered.

2. I am aware of the risks involved with the care outlined and understand the practitioner will do their best to provide the highest quality healthcare available.

3. I give consent to the delivery of the proposed management plan by the practitioner.

4. I give consent to release my medical history, healthcare results, and clinical notes to other affiliated healthcare professionals where deemed appropriate.

5. I am aware adequate notice must be provided for any appointment changes, or the following consultation fee will be charged. 48 hours from time of confirmed appointment schedule will incur a forfeiture of 50% of fees paid, while 24 hours or less notice will incur 100% forfeiture of fees paid.

6. I understand and agree to pay in full, any and all services rendered to me by my healthcare provider.

Home Visit Medicine: Acupuncture and Chinese medicine emphasize correcting habitual patterns that may be impacting one’s life; Reinforcing the need for improved nutrition, elaborating upon the many benefits of regular activity and adequate rest, while also promoting awareness to take charge of one’s own health and well-being.

Our belief is if an individual leads a modest and healthy lifestyle, both mentally and physically; they will remain more resilient to injury and illness, preventing the effects of ill-health and disease.

 

Application of Terms of Use
